Required documents

To start your rotation at Jackson Park Hospital, you will need to provide the following documents. Our team will guide you through each one.

  • Valid photo ID (passport, driver's license, or state ID)
  • Resume
  • Letter of good standing from your medical school or affiliated medical education program
  • PPD (tuberculosis) test within the last 6 months
  • Physical exam from a primary care physician (within 6 months to one year)
  • Proof of immunization to hepatitis B
  • Proof of immunization to measles, mumps, and rubella (MMR)
  • Flu shot proof
  • Criminal background check
  • OSHA, HIPAA, and Basic Life Support (BLS) for Health Care Providers certificates
